Responsibilities:

Assist in evaluating clients’ needs and developing appropriate service plans
Support clients with accessing community resources (housing, food, transportation, employment, etc.)
Document client interactions and maintain accurate case records
Help coordinate appointments, follow-ups, and referrals
Participate in home visits or facility visits as needed
Collaborate with social workers, healthcare professionals, and other service providers
Perform administrative tasks, including filing, data entry, and compiling reports
Help advocate for clients’ rights and ensure services are delivered equitably
Maintain client confidentiality in compliance with HIPAA and agency policies

Qualifications:

Bachelor’s degree in Social Work, Psychology, Sociology, or related field
Prior experience in social services or human services is preferred
Familiarity with California social service systems is a plus
