Job Summary:

Responsible for overseeing and coordinating all aspects of clinical and administrative operations within our home health care division in accordance with applicable law and regulations. You will play a crucial role in ensuring high-quality patient care, regulatory compliance, and operational efficiency.

  Essential Functions:

  • Develop and implement strategic plans to achieve organizational goals and objectives.
  • Provide visionary leadership to the home health care team, fostering a culture of excellence and continuous improvement.
  • Recruit, train, and develop a high-performing team of healthcare professionals.
  • Provide ongoing training and professional development opportunities to ensure staff competence and career growth.
  • Relationship Building: Establish and maintain strong, positive relationships with referring physicians, medical practices, and other healthcare stakeholders in the community.
  • Educational Outreach: Provide information and educational materials to referring physicians about our services, specialties, and the latest advancements in healthcare.
  • Identify Opportunities: Continuously identify opportunities for collaboration and partnership with referring physicians to increase patient referrals.
  • Problem Resolution: Act as a liaison between referring physicians and our healthcare facility to address any concerns, issues, or feedback, ensuring a smooth referral process.
  • Market Analysis: Monitor and analyze market trends, competition, and the healthcare landscape to identify potential growth areas and strategies.
  • Data Management: Maintain accurate records of interactions, referral sources, and outcomes to track progress and performance.
  • Collaboration: Work closely with our medical team, marketing department, and other relevant departments to develop and implement referral strategies.
  • Feedback Collection: Gather feedback from referring physicians to continuously improve our services and meet their needs.
  • Compliance: Ensure all interactions and communications with referring physicians adhere to relevant healthcare laws and regulations.

Additional Responsibilities:

  • Performs other duties as assigned or requested.
  • Conforms to all applicable Agency policies and procedures.
  • Participates actively in continuing education and in-services.
  • Maintains confidentiality of patient information and business trade practices
  • Assumes accountability for reporting incidents and complaints according to Agency policy.

  • Keeps abreast of changes in health care law.
  • Maintains Agency/program compliance with local, state, and federal laws as well as state accreditation standards.

Minimum Position Qualifications:

  • Education: Bachelor’s degree in Nursing, Healthcare Administration, Business Administration, or related field
  • Experience: 5 years of experience as a Registered Nurse within the Home Care setting. Minimum of 3 years of progressive experience in home health care management, with demonstrated leadership and managerial skills
  • License / Certification: Current State RN license; current CPR; current driver’s license and proof of auto liability

Environmental Conditions:

Works under a variety of conditions in facilities and offices; ability to work flexible schedule, ability to travel locally; some exposure to unpleasant weather. Moderate noise level; tasks may involve exposure to bloodborne pathogens; moderate stress and emotional demands.

Physical Requirements: Sitting is required. Requires ability to always handle stressful situations in a calm and courteous manner. Requires working under some stressful conditions to meet deadlines and agency needs. Ability to travel.
