Position Summary:

The individual in this position works under the direction of the RN and/or Social Worker to complete referrals for the post acute services for patients and assist with tasks that do not require a clinical license or degree. The individual's responsibilities include the following activities: a) arrange post-discharge services, b) create and follow up on electronic referrals using the Management system, c) review patient choice letters with patients/families for required signatures, d) provide follow up Important Message to Medicare patients prior to discharge, e) communicate with patients, families and other members of the care team, f) complete tasks assigned by Case Manager and Social Work staff, g) make copies, send faxes, and complete phone calls, h) complete process review or audits as requested, and i) other duties as assigned.

Required Qualifications:

• High School diploma or equivalent required
• Excellent organizational skills
• Excellent verbal and written communication skills
• Demonstrated problem solving skills
• Computer literacy
• Must possess the skill, knowledge and ability essential to the successful performance of assigned duties
• Must demonstrate customer service skills appropriate to the job
• Excellent written and verbal communication skills in English
• Ability to maintain a work pace appropriate to the workload
• Ability to establish and maintain effective working relationships across the Health System

Preferred Qualifications:

• Bachelor’s degree
• Paramedic, EMT or Nursing Assistant license
• Acute hospital experience
• Knowledge of University and medical center organizations, policies, procedures and forms
