Position Summary:

The individual in this position works under the direction of the RN and/or Social Worker to complete referrals for post acute services for patients and assist with tasks that do not require a clinical license or degree. The individual’s responsibilities include the following activities:

Required Qualifications:

• High School diploma or equivalent required

• Bachelor’s degree preferred

• Paramedic, EMT or Nursing Assistant license preferred

• Acute hospital experience preferred

• Skills required include excellent organizational skills, excellent verbal and written communication skills, demonstrated problem solving skills, and computer literacy

• Must possess the skill, knowledge and ability essential to the successful performance of assigned duties
• Must demonstrate customer service skills appropriate to the job
• Excellent written and verbal communication skills in English
• Ability to maintain a work pace appropriate to the workload
• Ability to establish and maintain effective working relationships across the Health System

Preferred Qualifications:

• Knowledge of University and medical center organizations, policies, procedures and forms
